FATE POST-PROGRAMME SURVEY (copy 01)

Dear ,We trust you and your business are doing well. As we seek to improve on our programme offering and provide bespoke support to our alumni, we are carrying out a survey on entrepreneurs supported in the last 24 months. We have developed this 5-minute survey to receive data and evaluate outcomes of our support based on your business performances since your programme. The data received will also help us evaluate the effect of COVID 19 on your business and what kind of support you would require from FATE Foundation over the next couple of years. The effectiveness of this survey is very dependent on the quality of information you provide, and we solicit your support to fill the survey as honestly as possible and with as much information as would be useful to us. Insights from the data received will be used for reporting to the board & management and enhancing our program design. We will be happy to share with you the highlights of the survey once completed.
